HOUSE RULES
Which rules apply for you and us within our practice?
  • For the proper course of events, we ask you to read the house rules of Fysio Westlandgracht.Treatments and fitness sessions take place according to agreement.
  • We ask you to be on time for your appointments.
  • If you are unable to attend, we request that you cancel at least 24 hours in advance (by telephone or e-mail).
  • Agreements that are not canceled or not fulfilled in time are for your account. You will receive a bill for this that you must pay yourself.
  • At the first appointment, we ask you to bring your valid identification, insurance card and/or policy document on which the insurance conditions are stated.
  • Our patients are responsible for adequate coverage through their (supplementary) insurance. If there is insufficient coverage, you will receive a bill from us, and you must pay this bill yourself.
  • For your and our hygiene, we ask you to bring a bath towel or towel with every treatment or training.
  • Street shoes are not permitted when using the exercise room and equipment.
  • For safety reasons, it is not permitted to take your children while training in the exercise room.
  • In the practice and exercise room, we request that you switch off your mobile phones.
  • Eating and drinking is in the exercise room and the equipment is not allowed.
  • A smoking ban applies throughout the entire building.
  • You can make complaints to your physiotherapist, via the complaints form in practice, or via the complaints procedure that can be found on the website of Fysio Westlandgracht.
  • We are affiliated with the KNGF and we treat the following rules and guidelines of these associations.
  • We use the Intramed electronic reporting system. The Data Protection Act applies to the registration of medical and administrative data. This law contains rules concerning the purpose of the registration, the nature of the data that is registered, the management of the data, the persons who have access to the data and the right of inspection by the patient.
  • We treat your personal and medical details with care following the rules of the Personal Data Protection Act.
