For the above objective (s) Fysio Westlandgracht may request the following personal data from you:

  •     Insertion
  •     Last name
  •     Initials
  •     (Business) Telephone number
  •     (Business) email address
  •     Sex
  •     BSN number
  •     Date of birth
  •     Data insurance
  •     Details GP / treating specialist

Your data will be stored by Fysio Westlandgracht for the above-mentioned processing for the period:
  • During the term of the agreement and then only in the administration for a minimum of 15 years.
Your privacy within our practice

  • No other patients are discussed in the presence of patients or third parties.
  • Patients' privacy is guaranteed in the treatment rooms. The rooms are all separated from each other. The treatment room cannot be viewed from outside.
  • Within the practice, agreements have been made not to enter the treatment room without the patient's permission.
  • On the request of the patient, the physiotherapist will leave the treatment room while the patient (un)dresses.
  • Within the practice, no visible patient information (e.g. transfers or information in a computer) can be seen by third parties.
  • If you believe that the practice does not handle your data correctly, you can make this known through the applicable complaints’ procedure within the practice.
