What personal data do we process?
Depending on your contact with us and your treatment, we may process your name and contact details, date of birth, Dutch citizen service number (BSN) where legally permitted or required, insurance details, details of your GP or referring healthcare professional, appointment and billing information, and information about your health and treatment. Health data are special-category personal data and receive additional protection.
Why do we process your data?
We process personal data to organise and provide physiotherapy care, maintain your medical record, communicate about appointments and treatment, handle billing and financial administration, comply with legal obligations, improve quality and handle complaints.
The legal basis depends on the specific processing activity. It may include performance of the treatment agreement, compliance with a legal obligation or – where legally required – your consent. Additional statutory rules for healthcare apply to the processing of health data.
Electronic patient record and patient portal
Fysio Westlandgracht uses Intramed for its electronic patient record and online patient environment. Through the patient portal you can register with the practice and, when made available to you, enter information or complete questionnaires. Data processed through this environment are used for your care and related administration.
We recommend providing medical data, detailed health information and documents through the designated secure patient environment whenever possible, or discussing them directly with your physiotherapist. For general questions, you can contact us at info@fysiowestlandgracht.nl. Please avoid sending more medical information than necessary by ordinary email.
With whom do we share data?
We share personal data only when this is necessary and legally permitted. Depending on your situation, this may include other healthcare professionals involved in your treatment, your health insurer for billing and checks within the applicable rules, and service providers that process data on our behalf, including suppliers of our electronic patient record system and digital services. We may also be required to provide data where a competent authority lawfully requires us to do so.
We enter into the legally required privacy and security arrangements with parties that act as processors on our behalf.
How long do we retain your data?
As a general rule, we retain your medical record for at least 20 years from the date of the last amendment to the record. In specific circumstances, a longer retention period may be necessary or legally permitted.
Other personal data are retained no longer than necessary for the purpose for which they were processed and for as long as applicable statutory administrative retention periods require.
Security
We take appropriate technical and organisational measures to protect personal data against loss, unauthorised access, alteration and unlawful processing. Access to patient data is limited to people who need those data for their work.
Your rights
Depending on the circumstances and applicable law, you have the right to access your personal data and medical record and to receive a copy. You may request correction of factually incorrect personal data and may ask for your own statement to be added to your medical record. Under certain conditions, you may request the destruction of all or part of your medical record. Statutory exceptions may apply to some privacy rights.
